Provider Survey to Update Data Remediation Point of Contact

NYSDOH has set up a customer service process for patients who are aware of issues with their vaccine records. Patients vaccinated by providers (i.e., not state-run mass vaccination sites) will be referred to their original vaccine administrator to resolve any data discrepancies, such as incomplete and inaccurate personal identifying information.
 
To facilitate this handoff between the state and your organization, we are asking for complete contact information of a primary point of contact and a back-up point of contact to whom NYS call center representatives can transfer patients.
 
If you would like to update your contact information, please fill out this form. If you would like to update contact information for multiple provider IDs within a network, please fill out this form once for every provider ID. Updates will be reflected in our database on a regular basis.
